Job Title: Nurse

I am looking for an enthusiastic Registered Nurse to provide onsite nursing services to older adults served by a particular NNORC (Neighborhood Naturally Occurring Retirement Community) Program on site Tuesday’s 8:30am-4:30pm with one-hour lunch.  Reports to NNORC Program Director, works collaboratively with NNORC staff and community partners, and follows guidelines of organization.   

This is an immediate opportunity. Requirement:  Must be bilingual in English and Chinese (Mandarin)


  • Assesses patients physical and psycho-social needs, utilizing tools provided by funding agencies and sponsoring organization, in order to design a comprehensive goal-directed plan of care and refers to other team members based on patients’ needs and lifestyle.
  • Collaborates with other members of social services staff and with other health care professionals in an effort to develop individualized care plans for older adults.
  • Provides information on community resources to assist the older adult in making appropriate health care plans.
  • Works collaboratively as part of a multidisciplinary team with other staff
  • Demonstrates sound judgement and independent problem-solving skills in order to initiate appropriate interventions with regard to patient's social, psychological, and or physical impairments.
  • Provides in-services and educational information to older adult and staff in order to broaden their knowledge of existing resources in the health care field.  Works with NNORC Director to determine areas of focus based on analysis of Community Needs Assessments and other relevant data
  • Promotes and maintains the health of older adults through teaching and counseling.
  • Provides health screenings, including blood pressure monitoring in order to maintain and promote good health amongst older adults.
  • Serves as a liaison with community agencies to facilitate the process of referrals and ensure linkage with needed community services.
  • Maintains standards of practice consistent with the philosophy, objectives and policies.
  • Administer and evaluate health indicator and other surveys as required. 
  • Complies with requirements of any grant-funded programs of the location.
  • Participates in Program staff, team, partnership and other meetings as required
  • Complies with documentation requirements including entering notes and units into program’s data base system.


  • New York state R.N. license
  • Proof of Covid-19 vaccine
  • Experience working with older adults preferred
  • Knowledge of health issues and concerns of aging
  • Understanding and appreciation of role of race, religion, ethnicity, sexual preferences and individual values as it relates to serving clients and their families
  • Ability to coordinate care with community health care providers as indicated
  • Ability to work independently and also work as a member of a team
  • Ability to work in a fast-paced environment and respond to difficult situations calmly and professionally
